As far as I know you need Mk2 stub axles, carriers, and calipers, plus Sirocco 16v handbrake cables. All parts should bolt straight on.
I've heard of people using Mk4 calipers, but I'm not sure whether they'll bolt to the Mk2 carriers.
There's a guy on here selling the whole conversion at the moment - have a look in the Parts for Sale section

mk4 claipers are better than the mk2/3 ones which are prone to jamming up (avoid the early mk2 calipers where the handbrake cable comes over the top..) latter mk3 calipers have an internal hand brake spring, which is better but the daddys are the mk4 calipers, where are alloy so lighter too
they will all bolt up to the mk2 carriers, and if using mk4 you will need the braded rear hoses from C&R, who also sell on ebay as they have a banjo bolt fitment.
the other bits are as listed.. plus you will need rear discs and rear wheel bearings too which will need pressing into the disc.
also a small bit of the end of the rear beam will need grinding to allow clearance for one of the bolts.
its a good conversion but not cheap, budget on ?200 min or so
