Coilover adjustment rings locked up wheel

Hi All,
I had a near miss yesterday whereby one of the front wheels departed the car! We obviously dropped a ball by not checking that the wheel nuts were tight prior to going out on the road….. as we have been restoring the car over the winter and this was first time out. However on closer inspection it looks like the adjustment rings on the shock absorber travelled down and caught and locked up the wheel, this assisted in causing the wheel to come off. I have no previous experience with coilovers so I'm wondering if I've missed something in terms of setting them up.
Any thoughts greatly appreciated.

May need spacers as wheel could of been to close due to offset of wheel

Did you lock the coilover rings against each other?

As said, may need spacers to clear the rings too

Cheers. The wheels that were on were just a  temporary set that came with the car ..15*7s. I put a pair of ATS cups 15x7s on today.
Brad.. I cant remember to be honest. I probably didn't pay enough attention to locking the adjusters…is it common for them to move even if they are locked?

If there locked together then no mate, should be no movement at all buddy.
