Water leak!!!!

So yesterday was so lovely and hot, I took the cab out for a long drive, all was going well, until I got about 200 yards from my house coming back from my drive and I had some smoke from the engine area, so I pulled over and I could see water was leaking from the top screw of the Plastic hose flange, on end of cylinder head which connects to the heater hose. Now what is weird, this was a new item with new studs in the head and nuts. So a slow drive back to the house to let it cool down.

Has anyone had this before, I'm going to take off the plastic hose flange today and have a look and see what the problem is…

I replaced plastic hose flange and with new nuts and fixed the leak

Those things have abismal failure rates. Every one that I see is leaking/cracked/or warped.
